This is a working draft for review. Bracketed [PLACEHOLDERS] must be completed and the full text verified against a real accessibility audit before this page is published.
Y-Factor Health is committed to making our website and at-home male fertility testing services usable by everyone, including people with disabilities. Accessible care is part of good care, and we treat accessibility as an ongoing responsibility rather than a one-time project.
We aim to conform to the Web Content Accessibility Guidelines (WCAG) 2.1, Level AA, published by the World Wide Web Consortium (W3C). These guidelines explain how to make web content more accessible to people with visual, auditory, motor, speech, and cognitive disabilities.
Current conformance status: [PARTIALLY CONFORMANT] — parts of this site do not yet fully meet WCAG 2.1 AA. Known gaps are listed in Section 4 below, and remediation is in progress.
Accessibility extends beyond our website. Because our clinicians come to you, we can often accommodate needs that a clinic visit cannot.
We are aware of the following limitations and are actively working to resolve them:
Some content on this site is provided by third parties, including our payment processor and laboratory partners. We ask our vendors to meet the same standard, but we do not control their code. Where a third-party tool creates a barrier, we will provide the same information or service through an accessible alternative on request.
We welcome your feedback on the accessibility of Y-Factor Health. If you encounter a barrier, or need information in a different format, please tell us — it helps us prioritize fixes.
Please include the page address, a description of the problem, and the assistive technology or browser you were using. We aim to acknowledge accessibility reports within [N] business days and to provide a substantive response within [N] business days.
If you are not satisfied with our response, you may escalate your concern to [PRIVACY / ACCESSIBILITY OFFICER NAME AND CONTACT]. You may also have rights under the Americans with Disabilities Act (ADA) and Section 504 and Section 1557 of the Affordable Care Act, including the right to file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
We review this statement at least annually and whenever we make substantial changes to our website or services. This statement was prepared on [PREPARATION DATE] and last updated on [LAST UPDATED DATE].
